  • Types of cases required: ENDO, OB/GYN, PEDs, URO, ENT, Ophthalmology, ORTHO
  • Must be proficient in Spinals, Blocks, Epidurals
  • Case logs required - proof of epidurals, spinals and blocks
  • Number of providers during the day - 1 MD / 3 CRNAs
  • Number of OR’s - 5
