Hematology - Oncology

VeloSource has a Hematology/Oncology Opportunity - Coos Bay, OR

• Minimum 2 weeks a month - REQUIRED
• Setting: Outpatient - Free standing cancer center
• Schedule (Shifts and Hours): 4 or 5 days per week, 8:30a-4:30p, admin day or 1/2 day if needed
• Call Schedule: 7 days of call including one weekend, 1 in 4 rotation
• Call Ratio: 1:4
• Number of Physicians in Practice: 3 currently
• Trauma Level: 3
• Board Certified in BOTH Hematology and Oncology - [REQUIRED]
• Active OR license - [PREFERRED], WILLING TO ACCEPT PROVIDERS NEEDING TO OBTAIN 
• Clean background/no malpractice - [HIGHLY PREFERRED]
• DEA - Verbal Confirmation [Required] at Name Clear
• Self-Query NPDB (Should be pulled within the month) [REQUIRED]
