• Patient Contact Hours: 6 hours/day
• Clinic Hours: 8a-4p
• Procedure/OR Days: 0
• On Call: Yes; exact schedule TBD
• Inpatient Responsibilities
• Outreach ~ 6 days per month
• Telehealth
• Job Description: Medical Hematology/Oncology coverage for 4/5 days per week. Inpatient coverage of 2 sites.
• Number of Physicians: 1
• Number of Advances Practice Providers: 0
• No Supervision of APPs
• Nursing Support: 1:1
• Medical Imaging onsite
• Lab & Pharmacy onsite
• Clinic Provider Daily Volume: 10-30
• Average Daily Hospital Practice Census: 10-15
• Appointment Length: consult - 40 minutes, follow up - 20 minutes
• Patient Population: Primary Clinic - Medicare/ Medicaid/Commercial; Secondary Clinical - Commercial, Medicare
• Services offered in the practice: Patient assessments, new treatment plans, ongoing treatment plans, changes in therapy, referral for ancillary services, end of life care, chemotherapy management, pain management (radiation and surgical oncology management provided by existing providers with strong relationships)
• Board Certified or Truly Board Eligible in Hematology and Oncology - [REQUIRED]
• Active IA License or Active IMLC - [REQUIRED]
• Fellowship Trained in Hematology & Oncology - [REQUIRED]
• Must be experienced - [REQUIRED]
• Clean Malpractice/Background - [REQUIRED] - Self-Query NPDB (Should be pulled within the month) at time of presentation
