• Reason for Coverage: New service line 
• Patient Population for Required Coverage: Adult, Geriatric
• What Percentage of the Practice is: Hematology 50%; Oncology 50% - 0% IM 
• Average Number of Patients Per Day - OUTPATIENT (Clinic/Office): 30 average
• Average Number of Patients Per Day - HOSPITAL (Call/Rounding): 0
• Clinical Area: Oncology - Outpatient Setting
• Required Procedures: Mgmt. of VAD/CV Catheters, Bone Marrow Biopsy
• Required Duties: Participation on a Tumor Board, Mgmt. of Bone Marrow Transplants, Orders Tests, Blood Bank, Orders Images, Prescribes
• Administration of Chemotherapy: Physician, Nurse oncologist
• Board Certification (provider must have AMBS/AOA board certification in one, and competency in the other demonstrated) - [REQUIRED]
• Active IN State License - [REQUIRED]
• BLS - [REQUIRED]
